Step 1 - Fill out the forms:

Complete your visa application form, the DS-156. This form must be filled-in online and then printed out in hard copy for your signature. Don’t forget to sign on the second page!   This form is for all nonimmigrant visa applicants.

All applicants for F, M, or J visas also need to complete the DS-158. This form can just be completed by hand.

In addition, all male applicants aged 16 to 45 must complete the DS-157.
