Final Checklist:

Before you attend your consulate interview please review this checklist to ensure that you have everything required.

  • DS-156 Form – Completed online, printed and signed
  • DS-157 Form – Males Only, aged 16 to 45
  • Valid passport
  • Visa application fee receipt – payable at any NZ Post Shop prior to your interview
  • Pre-paid, self-addressed "track and trace" courier bag
  • One passport sized photo
  • Supporting documents
  • If applicable, your Criminal History Report. You need this only if you check “yes” in block 38 of the DS-156.

For F, M and J visa applicants, also:

  • DS-158 Form
  • SEVIS fee receipt
  • DS-2019 form for J visas or I-20 form for F and M visas
  • Acceptance letter from your visa sponsor, if applicable
  • Copy of your job letter or placement letter in the U.S., if applicable.
  • If your particular program requires that you are enrolled full-time at a tertiary institution, a letter from your institution confirming your full-time student status or a copy of your transcripts (unofficial web version is fine.)

For H, L, O, P, Q and R visas:

  • Copy of an I-797 approval notice
