Passports

Damaged or Mutilated Passport

To obtain a new passport, you must apply in person to the Auckland Consulate at Level 3, Citigroup Center, 23 Customs Street East, Auckland (cnr. Commerce St).  Consulate location map

You will need to:

  1. Complete "Application for Passport", Form DS-11.  Please note that some of the instructions apply only to U.S. citizens residing in the U.S.
  2. Present Proof of U.S. Citizenship
  3. Present damaged passport
  4. Present Proof of Identity
  5. Provide Two Passport Photos
  6. Pay the Applicable Fee
  7. Provide a Social Security Number
  8. Complete a DS-64: Statement of damage.
  9. Provide a courier envelope if your passport is to be mailed to you.
