Passport Amendment

Extra pages:

Effective September 26, 2005, as an additional security enhancement, Passport Services will no longer amend valid passports. Instead, customers requesting name changes, extension of validity for limited passports, or correction of a printing error will apply for replacement passports. Media Note (PDF file; 12kb).

To add extra pages to your passport, you will need to complete form DS-4085.  You can either:

1. Complete the online version of form DS-4085 using the convenient Passport Application Wizard. You will need to be prepared to print the completed form, which includes a barcode that allows for more efficient processing of your application. If you experience problems while using the wizard, please use the manual method described below.

2. Download form DS-4085, print it, and complete it manually.

Once you have the form DS-4085 (completed either online or manually) you will need to send it, together with your passport, to either:

Include a prepaid courier pack for return of your passport.
Please allow a week to process your passport.  There is no fee to add extra pages to your passport.

Passport name change:

If your passport was issued less than 12 months ago please submit:

• A U.S. Passport Re-Application form DS-5504
• The passport that needs the change
• The original of your marriage certificate or your name-change court decree
• New passport photos
• A prepaid courier pack for the return of your passport.

If your passport was issued more than 12 months ago please submit:

• An Application for Passport Renewal form DS-82
• The passport that needs the change
• The original of your marriage certificate or your name-change court decree
• New passport photos
• The applicable fee(s).  If the requested data change is due to a printing error by Passport Services, there will be no charge
• A prepaid courier pack for the return of your passport.

Photocopies and notarized copies are not acceptable for passport purposes.

If your name has changed by any means other than by marriage or court decree, you must apply in person.
